About our Quality of Life survey

Tracking your quality-of-life can help you – and your doctor – understand the impact that treatment such as prescription drugs and/or medical cannabis has on your quality-of-life.

Our online tool is an adaptive version of the World Health Organization’s (WHO) Quality-of-Life (QoL) survey.

The survey takes about 10 to 15 minutes to complete. You should take the survey every 2 weeks; it asks you to think about your life over the last two weeks.

Your participation is completely voluntary, and you may withdraw at any time by simply existing the survey. Your information is anonymous and may only be used in an aggregated and anonymous manner such that you will never be personally identifiable.

What is an access code?
Some groups of people
or organizations have their own unique access code. For example, a health charity group may provide an
access code for their members. This allows us to group together those individuals.
